Families search “Aid and Attendance” and land on two different VA programs that share a nickname. One is a needs-based add-on to VA Pension. The other is a function-based rate under Special Monthly Compensation — SMC-L. They are not interchangeable. They pay differently. They use different rules.

Pension Aid & Attendance is not SMC-L
VA Pension Aid & Attendance is a needs-based benefit. Wartime service, income, and net worth matter. Surviving spouses sometimes qualify. That is a real program. It is not what a 100% disabled veteran is usually looking for when they say they need help bathing, dressing, or staying safe at home.
SMC-L Aid & Attendance is extra compensation paid because of service-connected disability and the need for regular help from another person. It is not a poverty test. Veterans already rated 70, 90, or 100 percent are often the people this rate was written for — and they are the people a 5×-the-increase business model tends to turn away.

100% is not the end of the conversation
The 0–100% table answers how much a condition impairs earning capacity. It does not answer whether someone else has to help you through a normal day. If the rating letter never looked at that, monthly pay can sit on the 100% row while the household is already doing Aid & Attendance work.

Housebound is a different letter
SMC-S (housebound) is not the same as SMC-L. Statutory housebound can apply when a veteran has a 100% rating plus a separate 60% — even if they still leave the house. Factual housebound is about substantial confinement. See VA housebound benefits (SMC-S).
What to do next
If a spouse, adult child, or aide is already helping with bathing, dressing, meals, medication, or getting around because of service-connected disabilities, say so in the consult.
